Make the visit useful to you
Think about what you want to understand before arriving. You may have questions about prevention, ongoing conditions, family history or changes since your last appointment. Share those priorities so the physician can help organize the conversation.
A preventive visit is not a promise of a standard testing package. Screening and other decisions depend on your circumstances and clinical assessment. Ask what is recommended for you and why.
What should you bring?
Check with the office beforehand if paperwork, fasting or a particular examination is involved. Instructions should come from the practice for your actual appointment rather than from a generic online checklist.
- Your current medicine and supplement list.
- Relevant records and recent results you already have.
- Information about prior screening or preventive care.
- A short list of concerns, changes and questions.
- Any school, sports or other form you need the practice to consider.
Physical, sports physical or Medicare wellness visit?
These terms can describe different purposes and requirements. A sports physical is tied to participation paperwork. A general preventive visit is an opportunity to discuss broader primary-care needs.
Medicare describes its Annual Wellness Visit as a prevention-planning visit, not a routine physical examination. That distinction does not establish that Cibolo provides or bills a Medicare-covered visit. Ask the practice about its membership model and the specific appointment you need.
Leave with a clear next step
Ask how any results will be discussed, what follow-up is needed and which parts of care take place outside the practice. Outside laboratory work, imaging or specialist visits may have separate costs.
Prevention continues between visits. An ongoing primary-care relationship gives you a place to revisit questions and discuss changes without treating every appointment as the start of a new health history.
Membership and outside costs
Cibolo Family Medicine uses a Direct Primary Care membership model. Adults are $165 per month each. Children are $35 per month each when at least one adult is included; a child-only household pays $95 per month for each child.
Membership is not health insurance. Outside laboratory work, imaging, prescriptions, specialist and hospital care, and some tests or procedures can involve separate costs. Ask the practice what is included for your needs before enrolling.
