What can ongoing diabetes primary care support?
Diabetes care is individual. Monitoring, medicines, everyday habits and other health concerns may all be part of the conversation. Regular communication helps you understand the plan and raise questions about what is working.
Cibolo's family-medicine physicians provide primary care. That is different from specialist endocrinology care. The appropriate care team depends on your diabetes type, other conditions and clinical needs.
- Reviewing your health history and current medicines.
- Discussing monitoring and follow-up with your physician.
- Considering nutrition, activity and other practical parts of daily life.
- Clarifying when outside testing or specialist support is needed.
Prepare when establishing or transferring care
Bring your medicine list, relevant records and any monitoring information your current care team has asked you to keep. Tell the practice about existing specialists and other clinicians involved in your care.
Write down what you need help understanding. This might include your follow-up plan, how to obtain records, possible outside costs or how to contact the physician with a non-emergency question. Your own treatment instructions should come from your care team.
Make the next step clear
Ask what should happen after the visit, when the plan will be reviewed and whom to contact if you have questions. The timing and type of tests or appointments should reflect your individual needs.
An ongoing relationship also provides space for preventive and other primary-care questions. Bring up changes in your circumstances so your physician can consider them in the wider health picture.
Know when other care may be needed
Some diabetes needs require specialists, hospital care or services outside a primary-care practice. Your physician can help explain the role of referrals and coordinate appropriate next steps.
Membership is not health insurance and does not replace emergency care. Ask about laboratory, medication and specialist costs before joining. For urgent or emergency needs, use the appropriate urgent or emergency service rather than a membership inquiry form.
Membership and outside costs
Cibolo Family Medicine uses a Direct Primary Care membership model. Adults are $165 per month each. Children are $35 per month each when at least one adult is included; a child-only household pays $95 per month for each child.
Membership is not health insurance. Outside laboratory work, imaging, prescriptions, specialist and hospital care, and some tests or procedures can involve separate costs. Ask the practice what is included for your needs before enrolling.
